This output form, selected on the Output Forms combo on the Print Reports page of Christian Learning prints a simple listing of all topics for the learning experience you select from the drop-down combo box directly under the Output Forms combo.

If you wish to print any notes saved with the selected topic, check the Include Comments/Notes check box.

Electing to Include Comments/Notes changes the composition of the form to accommodate the additional information.
